u should include the patient's age, from the age you still need to state what category they belong to (infant, elderly, middle-aged), gender, diagnosis, previous history, symptoms, test results, treatment/therapy recieved so far and the outcome of that so far (for example change or difference seen since treatment started), what diagnostic steps were taken to develop this diagnosis.here is an example of the opening of a case study:
Patient - A 70-year-old elderly man with a previous medical history of chronic bronchitis, alcohol abuse, and macrocytic anaemia. At this time the patient refused a bone marrow biopsy.
He subsequently developed general weakness, light headedness, tired all the time (TATT), fever without chills, night sweats, and a worsening of his chronic bronchitis symptoms. These symptoms were presented to the GP by the patient.
The laboratory test results: Revealed significant anaemia (Hb: 5.0 g/dL, HCT: 15.1 %).
At this stage he was admitted to hospital, where he recived transfusion therapy and underwent a bone marrow biopsy. Following transfusion with packed red blood cells a complete blood cell count (CBC) was performed and the blood smear revealed a macrocytic normochromic anaemia.

I've got an older, 4th edition, APA publication manual in front of me and the best I can come up with is to cite it in a manner similar to that of unpublished raw data....If the title is unknown and you are using a description of content, here's an example....
Bordi, F., & LeDoux, J. E. (1993). [Auditory response latencies in rate auditory cortex]. Unpublished raw data.
If the title is known underline it instead of using the brackets.
