Not a lot of information to go on here, but case studies usually follow a certain format. Assuming that you need to pick a subject for your study, or maybe even assess someone you know? Start the report by stating the "complaint" from the subject, or the subjects view of the "problem". Then follow with section describing their background information, this should include demographics like age, gender, sexual orientation, race, culture, ethnicity, place of birth, etc. Follow up with family background and dynamics. Then current functioning, how are they going about their lives now? Do they have a job? etc. Next, the symptoms, what the heck is wrong with this person that is observable (i.e., he washes his hands 30 times a day). Followed this with your analysis of all the information in previous sections, putting the information together and forming your opinion on the subjects problems and strengths (remember that any case study should include the subjects strengths and not just be focused on negatives). This is generally the longest and most difficult part, because it requires thinking rather than just data collection. Follow that with your diagnosis (from the current DSM) and conclusion. And finish with your recommendations for treatment. Hope that helps and good luck.
