To accurately diagnose, I'd need the normal values for the patient. Spirometry is dependent on the patient's age, race, and gender. Most of the values for those are calculated using a spirometry computer. But, based on clinical presentation...1) Emphysema
2) Obstructive. Slow expiration as well as low Max expiratory flow indicates an obstructive disease.
3) Emphysema is due to alveolar damage and resultant small airway obstruction. Bronchodilators can have some effect in dilating these small airways, but are not nearly as effective as if it was an airway disease such as bronchitis and asthma.
4) Destruction of the alveoli and hyperinflation in emphysema causes shifting in the lung zones from Zone 2 => Zone 1. This results in the compression of the capillaries and increased V/Q ratio. This increased V/Q mismatch results in hypoxemia and an elevated A-a gradient.
5) Before: 3.0, After: 2.8.
6) <10% response to bronchodilator does not warrant therapy, but does not exclude bronchodilator therapy. Altered RV is due to increase of 200 ml in the vital capacity.
7) Before: 412.5 ml. After: 421.875 ml.
8) TV is below normal. Normal tidal volume should be around 500 ml at rest.
