From your description, it surely sounds like you are dyslexic. That is definitely a big problem in learning, but I can tell you that you don't have a WORSE problem.
I work with students with all kinds of learning disorders and the worst problem to deal with is the one who just does not care! You care or you wouldn't have bothered to write this and take all the time you did to do it. I'm impressed.

Talk to your teachers and your counselor. If you show half the determination you did here, he/she will bend over backwards to do all they can to help you.

The reading comprehension cluster on the WJ-III Achievement Battery or any other achievement subtest cluster or composite score is just one part of determining whether a student has a learning disability or not.

Tests that look at a student's intelligence/learning potential as well as abilities in various different areas of psychological processing (visual and auditory memory, attention, organization, phonemic awareness, visual-motor integration, etc.) are also key components.

With all of that information taken together, a profile of strengths and weaknesses will emerge and depend on those patterns coupled with discrepancies between intelligence and achievement, a learning disability may be diagnosed.

I wouldn't go to U of Phoenix. They don't have a good reputation. It will cost you a lot of money and they don't have the same resources to help you with your disabilities that a real school does. Go to a state school; they are everywhere in California.
