As far as the Reading Log goes, I've done those when I was in high school and it's pretty simple.
So you keep a diary, this one needs to be typed, as you're reading the novel and whenever you come across a quote, whether it is something a character said, or something the author wrote that you really like, or maybe it spoke to you, or you like the way it sounds, .... you type that quote and then elaborate on the meaning, why it is important to the story, or why it was important for that character to say/think it...maybe that quote really defines that character's personality and how they act toward others. Or maybe the quote is one that changes the course of the novel or impacts the storyline, impacts the characters in some way. It sounds like you don't have to write a whole lot for that reading log, but you should try to write a paragraph for each explanation.

I've never heard of the term, Thought Paper, but I'm guessing it's shorter than an essay and not as restrictive (you don't need citations). So as you're reading, if you find a symbol or theme you can get at least a page worth of thoughts on, you should be good. You can probably google themes or symbols for whichever novel you choose, if you can't think of one on your own, just make sure you don't copy. :-)
In the thought paper, you'll anaylze a theme or symbol from the novel and give your thoughts: how does it affect the characters or make them act the way they do, do you agree/disagree, what would you have done, how is it (or isn't it) relevant in today's society?

Adult ADD and ADHD are very common and treatable.

Before seeing the doc, try to figure out what you are thinking about in these circumstances. Do you lack confidence ? Many people who do are trying to figure out what to say when the silence starts instead of listening.
