First, slow down and breathe. If you're at the top in everything else, then I honestly think your nerves are getting the best of you in math.

You said it was the comprehension, not the computation. That tells me you know HOW to do the problem, but you have trouble deciphering the wording. First, read the problem through once without trying to solve it. Can you paraphrase it? Put it into your own words?

Next, read it through a second time and underline the actions you're asked to perform (percent, how many left, area, etc).

Read it through a third time an cross out any extra information that doesn't relate to the equation you need to solve the problem.

I know that's a lot of reading, but it will help alleviate your concerns of not understanding the problem.

From the way you worded your question, I don't think you have an issue with doing the math. I think the wording of the questions is what's throwing you. Pay close attention to the math terms being used. You know what to do with them. Also pay attention to the answer possibilities. They will help you know if you're on the right track.

If your reading it for revision, whenever see something you don't understand. Write it down then do a mind map on the word/topic to see how much you can get for it.

try working out new problems everyday. Dig your mind at it. As for improving the reading comprehension skills, try reading newspapers out loud and/or anything that suits your taste. Hope this helps!!
