You may want to look at individual comprehension skills. For example, Helping Students Locate Main Ideas, Distinguishing Fact from Opinion, and Making Inferences are topics you can investigate. You can find a lot of articles online and in libraries, and some teachers can also give you some suggestions for specific comprehension strategies. Good luck.

Reading is very important as well as clear writing. A tutor might help but taking classes in reading might help also. Maybe you could ask a English teacher to evaluate why you have a problem so you could figure out what you need to learn.

Absolutely. Any distraction, and vertigo could certainly be considered a distraction, is going to affect you ability to process and comprehend what's going on around you. If you can get the vertigo under control, I suspect that your comprehension will improve immediately.

It's a little hard to answer this question when your age is unknown. You are probably dissecting things you read too much. Generally, if you read a passage, it's best to read the whole thing at once. You will probably get the "gist" of it if you read through things all at once.

As far as metaphors go, it is really just a matter of hearing them several times before you allow your mind to sway off the "linear" track that your mind apparently works on . For instance, you probably readily understand the metaphor "light as a feather". You get the imagery of a feather being light so if I told you that my mother was on a diet and had become "light as a feather", you would understand what I was saying,(I hope).
Some authors tend to be very obscure in their writing and if YOU don't understand their metaphors, then probably many other people don't either.

find books he is interested in... you want him to read for pleasure, not because he has to... Try Captain Underpants, Harry Potter, magazines like Zoobooks, National Geographics Kids, Boys Life, and even comic books work at the beginning...maybe Graphic Novels? these suggestions should work, but its up to you to figure out which books are at his reading level from what ive recomened...

Also just informally talk about the book...ask him his favorite character, why, to describe the setting, what was the problem, how the author could've made it more interesting, etc... Then later after a month of this, switch these informal conversations into formal questions...have him dig deeper...why do you think the character did this>?what would you have done? How did he probably feel? etc...also make connections and infer...think up posible solutions after finding the problem...make predictions before reading when looking at the cover and pictures if there are any...GOOD LUCK
