Science DVDs = Bill Nye the Science Guy handsdown...the kids love him and you can find topics on just about anything related to science...you can order his videos or DVDs from this site and also on Amazon I believe. Social Studies DVDs = Just the Facts series...these focus primarily on the government portion of 5th grade starting with the American Revolution...they are quite good and make the topics relevant to the kids.
Social Studies games = I suggest getting a book on social studies simulations from Scholastic like this one. I have used it multiple times and the kids always love the activities and get a lot from them.

I would also recommend using books written by Jean Fritz to help teach 5th grade social studies. She writes biographies on numerous characters and events from history in a fun and interesting way. My students do a project on her books every year and love it!
As for math, my kids love to play a variety of different mental math games from I Have, Who Has to Beat the Teach to Multiplication War. Here is a website with some great game ideas:
