You would write to the author, you would write to the publisher. Did you note the bibiliographal information about the work you want to adapt from?

If so, you would send a request to that company's Permissions Department with the publication details of your planned work, a copy of the adaptation as you want to use it, and the source information from their work.

If you used UPS or Fed Ex, they would probably go the extra mile for you and find the tracking number. They have lots of ways to do that. Unfortunately, USPS does not, their systems just aren't that sophisticated. Sorry to say, you are probably SOL on this one. It may turn up after awhile -it's not uncommon for it to take 8-12 to process a return but I can understand why it would be difficult for them to track down without a tracking number. Sorry.
