I think with your sons level of development being as low as it is he MIGHT qualify for SSI. You need a formal dx before anything. Ask your pediatrician for a referral to a "multidisciplinary evaluation at a child development clinic" at a University. You will want a "multidisciplinary evaluation" because your son has issues in several areas of his development. You want to go to a University because they are the most up-to-date when it comes to evaluations. Your insurance should cover it. If you don't have insurance you can ask the school for an "independent evaluation" and they should approve it. You will probably need to put your request in writing. An independent evaluation at a University is better than separate evaluations with several different professionals. This way ALL the profession as will be working together to come up with the appropriate dx for your son. Good luck!!

I usually avoid trying to list disabilities, because the list will never be complete - so I ordinarily stick to categories of disabilities. Even that isn't completely accurate as many disabilities don't neatly fit into only one category. I sometimes lump hearing and visual disabilities into sensory, but sometimes list them separately (but then other sensory disabilities are left out.) There is no perfect solution - but I do get your point.

Advisor? Are you in college? Your university should have a disability services office or a counseling center that may be able to get you in touch with the correct people to do the testing at a much reduced rate.
