I think the best way is to get a bunch of index cards and simply write down the names of the states on one side and the capital city (and largest city also) on other side. I love test like that.Good Luck!!!

Get a friend, or your parents to quiz you on facts you studied every 2 hours or so.Drink a little bit of coffee, as caffiene stimulates your brain on remembering some facts (short term memory).
Try to group specific facts with a particular word/image/thing that will help you recall the information while you take the exam.

As a matter of fact I'm studying for my paramedic exam right now! lol-- There are TONS of study guides, tools, self-exams, dvd's, audiobooks, PC programs...etc....I mean you name it, it's out there! If you enroll in the program you'll probably have to get the box they tell you....but If you just go to the bookstore and look in the medicine or nursing sections..you'll find something! Most study guides/exams are categorized by skill level. If your state requires a NREMT cert., I'd recommend checking out their website: nremt.org--I'm assuming you're new to this field and will want to look for EMT-Basic study exams. Focus and determination is key. When you do not know the material, study until you feel like your understanding of the concepts are completely fulfilled. If its a math course or anything similar you need to do as many problems as you can until they become easy. That's when you know you are ready. The best grade you can get is determined by how much effort you are putting into studying. You may be the average test taker but if you try hard enough you can become a straight A student.I understand you said science and social studies but this helped me alot in college for all my courses.
My personal tips are:
Read material before class or before teacher even teaches it.
Ask questions when you do not understand, schedule time with teacher if you are really lost.
Take notes, from the text, from the lectures, and soak it in
Practice problems until you can solve them correctly without any help (i.e. flipping through example problems and notes)
Get into the right mindset: Why do you want an A? What is it that you feel are you weaknesses when it comes to tests (are you confused on advanced problems or having trouble choosing the right multiple choice answer?) Focus on areas that you may be lacking, for those 2 subjects you probably are not memorizing all the key facts and concepts, study key dates and important notes, make sure you understand why and their significance. For science I dunno what you are learning but its all pretty much memorization.
