You would take the MCAT to get into medical school. I am fairly sure that you won't need further credentials after becoming a Canadian-trained doctor. Canada has relatively few medical schools compared to the U.S. (as it has 1/10 the population). Therefore, the schools are all held to a certain standard which exceeds that required by most American hospitals. In the U.S., however, there are some sub-standard schools as there are elsewhere in the world. Therefore, to work as a doctor in the U.S. you need to graduate from a school that meets these standards. Since all of Canada's medical schools meet this requirement, Canadian doctors can work in the U.S. In fact, one of the largest problems with the Canadian medical system is that you can make a lot more as a doctor in the U.S. so it was quite easy to lure Canadian doctors south, and American hospitals were confident doing so because they knew these doctors would be trained at a school where the standards were acceptable.

Personally speaking, I would think that going to school in Canada first would be easier. After your first semester (or year) of studying, you can apply for a part time job. Some majors offer co-ops that could lead to a permanent job after graduation. You need to work several years before you can apply for citizenship so working while going to school should help. If you hold a valid study permit until mid 2011 you might be authorized to stay until your permit expires, as long as your passport covers the validation. However you will need to get a re-entry permit should you plan to leave Canada again and re-enter after your visa is expired.

Probably. This is a little flaky but it falls between the cracks and you can probably get away with it. If you ar eoutside of the USA you do not need a visa to work for the US company but the H-1B will allow you to visit them as you require.

And Canada does not care if you work for a non-Canadian company. What they care about is that you do not take a job from a Canadian.
