I think for most of the school the answer is no. I say mostly, because I remember vaguely that once I heard that an international student had to stay on campus. I tried to remember which university that was...but I couldn't find it. But like I said, most of them don't have that requirement.
To be 100% sure call up the school you are interested in and ask away your questions.

And the thing about going to a WI uni first is not exactly true. It all depends on the requirements of the program you are applying to and the school (since different school will have different requirements). I'm going to assume that high school in the US is similar to high school in Canada (unless it was some kind of fancy school..hmm but even then...you should be okay me thinks)
It'll come down to the grades for the required courses.

To apply, you need to have a bachelors degree in something, have taken the premed courses in bio, chem, physics, math, and English, and have gotten a good score on the MCATs. Med school is an additional 4 years. They are located all over the country, and while tuition varies from school to school, expect it to be expensive ($20,000 - $50,000 per year).

I think universities are privately owned, but they operate under Provincial guidelines and can receive funds from the government (I'm not sure how that works).
